Disclosure of beneficial interest to seller (by a chattel auctioneer) form

Give this form to a seller if you have a beneficial interest in the sale, such as if the buyer is:
- yourself
- a member of your family
- your business associates or employees
- an executive officer of your corporation.
There are no fees to pay with this form. Penalties apply if you fail to disclose your interest.
Cooling-off period and statutory warranty (motor dealer or chattel auctioneer to buyer) form
Form 12 – Motor Dealers and Chattel Auctioneers Act 2014
Motor dealers must complete this form and give it to the buyer immediately before completing the contract of sale for a vehicle.
It includes details about:
- the cooling-off period
- the statutory warranty.

Replacing a licence document

You can apply online or you can contact us by writing a letter or email as soon as you notice that your licence is lost, stolen, damaged or destroyed.
Changing your licence conditions

Deactivating or reactivating

When reactivating a licence you may also be required to pay a pro-rata renewal fee depending on whether the licence has previously been renewed as inactive and also a criminal check fee. You will be advised of the required fee to pay by our office when the application is made.

Requesting a licence extract

The record will show:
- the name of each person who holds the licence
- their registered business name (if any) and place of business
- the status of the licence—current, expired, refused, cancelled or suspended
- their licence number and class.
